Healthy People
Goal: Promote personal safety, healthy behaviors and access to quality healthcare for all.
Here's what successful grant recipients will need to use for measuring their programs:
- Access to care index – average percent of residents with health coverage, percent who sought needed medical care (need care/got care), percent who sought needed mental health care, percent who have a usual health care provider, and percent who did not have difficulty affording prescription medication in the past year.
- Health care equality – average disparity between African American and White residents in infant mortality, low birth weight, first trimester care, teen natality, death rate, heart disease death rate, stroke death rate, and cancer death rate.
- Healthy behaviors index – average percent of residents who do not smoke, percent who do not drink heavily, percent who are not obese, percent who eat at least five servings of fruit/vegetables daily, and percent partaking in adequate moderate physical activity.
- Percent of unpreventable hospitalizations – percent of hospitalizations that were not classified as ambulatory care sensitive (preventable).
- Population in safe homes – annual cases of abuse, neglect, and domestic violence per 100 residents.
